Purpose
This guide explains how hiring managers can request the deactivation of accounts for an offboarding employee using the offboarding form available on the Human Resources SharePoint site.
Steps for Hiring Managers:
Access the Human Resources SharePoint Site
- Navigate to the Human Resources SharePoint site from your web browser.

Locate the Offboarding Form
- On the HR SharePoint homepage, find the US Quick Links Section.
- Select the US - Offboarding New Employee link.

Fill Out the Offboarding Form
- Complete all required fields, including:
- Employee Name
- Last Working Day
- Department
- Manager Name
- Ensure to specify which accounts should be deactivated/delegated (e.g., Email, AD Account, Office 365, Application Access, etc.).
- Complete all required fields, including:
Submit the Form
- After completing the form, click the Submit button at the bottom.
- You will receive a confirmation that the request has been sent to the IT team.
Track the Request
- You can track the status of the request by checking the IT ticket that is automatically generated
Follow Up if Necessary
- If there are any changes or updates after submission, such as a delayed start date, notify IT as soon as possible.
